Insurance and Billing
*THERE ARE MANY PLANS UNDER EACH INSURANCE NETWORK “UMBRELLA.” CALL YOUR INSURANCE COMPANY TO VERIFY THAT DR BRENDEN IS IN-NETWORK WITH YOUR PARTICULAR PLAN. THIS IS YOUR RESPONSIBILITY AS A PATIENT.
Please verify your insurance coverage with your insurance company prior to scheduling an appointment with Dr Brenden as a specialist.
Dr. Brenden is an in-network provider for:
Medicare: Traditional/original Medicare. Dr Brenden is a Medicare provider for traditional Medicare Part B with the red, white and blue card, as well as various Medicare Advantage Plans through other insurance networks. (See below, or check with your insurance company to verify.)

Dr Brenden is contracted with many (but not all) Aetna plans. Call the number on the back of your Aetna card and find out what your benefits will be if you see Dr Lloyd Brenden, Dermatologist (Specialist level).

If you do not have insurance, or have insurance other than these plans, you are welcome to see Dr Brenden as a self-pay patient. Payment is at time of service.

If you have an office visit copay, it is payable at check-in, at the Specialist level.

If you need an insurance referral from your PCP to see a specialist, we need to receive it prior to scheduling your appointment.

We do not call insurance companies. Your insurance coverage is between you and your insurance company.

Our billing office will bill any two insurances you wish.

If you have a third insurance policy, such as a cancer policy, after Medicare and your secondary have paid, call Billing at 480-985-1093 and ask them to send you a bill which will show your charges and payments. We will provide a copy of your pathology report. Send your insurance company your claim form, a copy of your bill, a copy of your pathology report, and a copy of your Medicare Explanation of Benefits.

If you have a balance you wish to pay by credit card, you may pay online (see below) or call our billing office (see below) or our main office number, 480-985-9492 during office hours.
